For New Patients

If you are a patient new to our practice, please complete the new patient form (available soon as a downloadable PDF file) and bring them with you to your initial appointment. Please review the following forms prior to your arrival, which will detail office policies, office hours, fee schedules, and more.

If you are seeing Dr. Kelly, it would also be in your interest to contact your insurance company before your intake appointment to learn about the details regarding your health savings-account information or out-of-network mental-health benefits and limits. The office will assist you in submitting a claim to your insurance company for possible reimbursement of fees paid.

For New Patients of Dr. Kelly

Dr. Kelly charges $300.00 for the initial appointment. This appointment will take approximately 2 hours. She cannot accept any patients with Medicare, and does not accept insurance payments. If you would be interested in obtaining reimbursement from your insurance company, please see the "Insurance Reimbursement" form on this page.

For Existing Patients

You may ask your doctor to register you for a Patient Portal, accessible at http://www.patientfusion.com, which will allow you access to some features of your medical records, online scheduling, email and text message reminders of upcoming appointments, and other perks.

For existing patients of Dr. Kelly, the cost of a follow up appointment ranges between $150.00 - $200.00 depending on the appointment length.

To Schedule an Appointment

To set up an appointment or to learn more, call the office Mondays through Thursdays from 9 a.m. to 5 p.m. Otherwise, both new and existing patients may schedule an appointment online at their convenience. After you schedule an appointment online, the secretary will follow up with you to verify the information and date.

The offices will be closed Friday. Any phone messages left on Friday will be returned the next business day.
